Participation Agreement Form

Important: For legal/liability reasons, all participants (along with parent/guardian, if under 18) must sign the Participation Agreement Form and email it to ryoko2@illinois.edu (or one of our other organizers) by 9:00 AM CT on November 14, 2026 to be eligible to participate in IMT 2026. The waiver is available below.
